• Description: Offers Enhanced Care Management and Community Supports programs. Enhanced Care Management(ECM): Offers extensive case management services that will address clinical and non clinical needs of clients. Clients will have a Lead Care Manager who will provide comprehensive case management. Lead Care Managers will help clients identify needs, create a care plan to address those needs, and assist connecting clients with doctors and resources. Community Supports(CS): Offers supportive services aimed at addressing the social and economic factors that impact health outcomes and contribute to health disparities. Community Supports available through ButteCARES: Housing Navigation Services - Clients experiencing homelessness or at risk of experiencing homelessness receive help to find, apply for, and secure housing. Housing Deposits - Clients currently enrolled in Housing Navigation services may also be eligible for assistance with housing security deposits, utilities set-up fees, first and last month’s rent, and first month of utilities. Deposit assistance awarded on a case by case basis. Housing Tenancy and Sustaining Service - Members receive ongoing support to maintain safe and stable housing, including assistance with landlord communication, housing recertification, and access to community resources to prevent eviction.

• Eligibility: To receive Butte CARES's services, you must be: A Butte County Resident Adults ages 21 and older or families with children under the age of 21 A member of Medi-Cal with Partnership HealthPlan of California Be unhoused or at risk of being unhoused, including individuals who have been served an eviction notice
